The Pathways Program by The Collaboration Center provides quality case management services and culturally sensitive support to all families members, caregivers, and providers on what resources and funding are available in our community.
9:00-10:00am: Group Meeting

10:00-11:00am: Chance to meet 1:1 with a social worker

Complimentary breakfast food & beverages will be served. Child care is available for $10/hr through Inclusion Fusion’s Day Program.

Health/Medical

Educational Advocacy
Mental Health
Occupational Therapy
Physical Therapy
